Inform us of the death and where the death has occurred. We will help co-ordinate service and cemetery arrangements.

Contact your Clergy

Discuss time and place of funeral service. Confirm with the funeral home.

REQUIRED INFORMATION / DOCUMENTATION
— Full name of deceased (including maiden name)
— Health card number
— Date of death, place of death
— Name of family physician or attending physician or coroner
— Birth date of deceased
— Birth place (land location if rural)
— Usual residence (land location if rural)
— Mailing address and postal code
— Martial status
— Spouse’s name (maiden name)
— Occupation
— Father’s name & birth place
— Mother’s name (maiden name) & birth place
— Choice of burial or cremation

SELECT DONATION RECIPIENT
Decide where donations may be given ie: local charity, provincial or national.

OBITUARY
Gather obituary information, including date and place of birth, education, occupation, memberships, service clubs, outstanding work, hobbies, list of predeceased family and list of surviving family. The funeral home can assist in writing the notice and submit it to the newspaper.

SELECT ASSISTANTS
Select pallbearers, honourary pallbearers, ushers or memorial guest book attendants.

SELECT A FLORIST
Consult with a florist for family flower arrangements. Plan for the disposition of flowers after the service possibly to a family residence, church, hospital or seniors home.

-Consider arranging for a family member or friend to answer the door and telephone and keep a record of calls.

-Arrange for childcare as necessary.
